Paper Form Registration 

You can register by completing a form available from reception. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery along with two forms of personal identification (one showing your current address and another preferably with your photograph on). If you have your old medical card, then please bring this with you as well.

Please click here to register with Liquorpond Surgery (Available in multiple Languages). Your registration will be processed and it may take up to 2 working days until you’re registered. You’ll receive a confirmation when your registration form has been submitted and when you’re fully registered with us.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

We recommend that new patients undertake a health check with a practice nurse.

How do I register as a temporary resident with a GP?

If you are not registered with a GP but need to see one, you can get emergency treatment at any GP surgery.If you need treatment for more than 14 days, you will need to register with that GP surgery as a temporary or permanent patient.

You can register as a temporary resident if you plan to live near the GP surgery for up to 3 months.

Registering with us after leaving the forces

You may have recently left the forces and wish to register with us. You will need to live within the practice boundary or already live with a family who are registered with us. When you leave the forces, you should be given the option of receiving your medical records for you to pass on to the GP Practice. Alternatively, you will be asked to complete the Military Medical Record Request Form (see below link), which should be handed in when you register with us for us to request your records from the military.

Requests for military medical records can take up to 6 months for us to receive your notes so we would advise that you get copies of your records before you leave so we can merge these with your NHS records.
